Call your nearest HealthReach location to schedule an appointment. Staff will help determine if you qualify for the sliding fee scale during intake.

HealthReach Community Health Centers is a network of 11 federally qualified health centers serving families in rural central and western Maine. Locations include Albion, Bethel, Bingham, Kingfield, Livermore Falls, Madison, Rangeley, Strong, and Whitefield. They offer an Affordable Care Program with a sliding fee scale based on income and family size.
