Getting started.
Who this helps
Children from birth through age 5 may be referred when there are concerns about development or a possible disability. Early Intervention serves children from birth through age 2. Part B provides free public special education for eligible children ages 3 to 5.
How to get started
Within three business days, a representative from the CDS site serving the child's town will contact the parent or guardian. They will ask for more information, discuss concerns, explain CDS, and ask whether the family wants to schedule screenings or evaluations to check eligibility.
Gather referral details.
Prepare the child's and family's information and the concerns you want CDS to review.
Send a referral.
Use the Online Child Find Intake Submission Form, call 877-770-8883, or fax the Child Find Intake Form to 207-624-6661.

A few questions before you go.
Have the child's name, date of birth and home address; who the child lives with; areas of concern or diagnosed conditions; parent or guardian names, mailing address and phone numbers; and the referrer's name and contact information. If known, include the child's primary health care provider and any other agencies working with the child or family.
- Can my child be referred before receiving a diagnosis?
- What details should I have ready for a referral?
- What happens after I send the referral?
- Will my child need a screening or evaluation?
