The clinic serves adults and children and prioritizes Lincoln County residents. Pediatric care is for children from birth through age 18. New patients can register for restorative treatment only.
How to get started
After the clinic receives your new-patient form, staff will contact you to schedule. Staff will also contact you after you submit a financial assistance application.
Register for treatment.
On LCDental.org, choose Patient Forms, then New Patient Forms. Complete the form to register for restorative care.
Apply for fee help.
On LCDental.org, choose Patient Forms, then Financial Assistance. Review the requirements and complete the online application if you qualify.
