Getting started.
Who this helps
Older adults and adults with disabilities may qualify. Most in-home services require an assessment of functional and financial eligibility.
How to get started
The assessment asks about the help needed and the person's financial circumstances. The agency decides whether a face-to-face assessment is needed. If it is, an assessor will contact you to schedule it. The assessment is private and has no charge.
Apply for long-term care.
Apply for MaineCare services using the Long-Term Care application.
Get help with the form.
If you need help completing the application, contact your local DHHS district office.
Request an assessment.
If you have not had a functional assessment by Maine's Assessing Services Agency, call 1-833-525-5784.
How they can help.

A few questions before you go.
Check how the service works, when it is available, and any costs. Ask about communication or accessibility support if you need it.
- Which home care program might fit my needs?
- What kinds of help can the programs provide?
- What will the assessment ask about?
- Can I direct my own care?
